Review the list of Qualifying Event Reasons below and please identify the type of Qualifying Event (QE) you have encountered. You must also provide documentation to verify that your qualifying event occurred within the past 60 days. Each qualifying event below indicates the required documentation that you must provide in order for your application to be complete. 1. Newborn child / adoption
2. Marriage / domestic partnership
3. Court order
4. New foster child
Loss of coverage due to:
1. Change in job status / Job loss
2. Group contributions ended
3. Child reaches age 26
4. COBRA ended
5. Divorce / separation
6. Loss of Medicaid / Medi-Cal
7. Moved out of service area
8. Medicare
9. Death
Other event:
1. Permanent move and/or new resident of CA
2. End of military service
3. Release from incarceration
4. Change in eligibility for financial help
5. Completion of covered services
6. Contract violation
